As at September 2026, the ServiceM8 AU pricing page lists Electronic Forms on the Growing, Premium and Premium Plus plans. Forms bought from the ServiceM8 Form Store work on every plan. So if you are on Free or Starter, a ready-made form is the only option until you move up.
We check your plan on the first call, so nobody pays for a form the account cannot run.

Build the questions and logic
Each question gets the right type: photo, signature, date, number, multiple choice or text. Questions that do not apply are skipped automatically.
Design the Word template
The answers merge into a Word template with your branding and layout, so the finished PDF looks like your paperwork, not a default printout.
Attach, test and hand over

A forms build starts from $1,800 + GST, fixed as one number after we have seen the forms you use now. One form is included in our Basic ServiceM8 setup at $4,800 + GST, and forms creation is part of the Power User package at $11,500 + GST. Both are listed on the ServiceM8 services hub. No hourly billing and no lock-in contract.

Can ServiceM8 forms produce a compliance certificate?
Yes. ServiceM8 describes forms being used for inspection reports, certificates of compliance and safety assessments. The answers merge into a Word template, so the finished PDF can follow the certificate layout your trade uses. You stay responsible for what the certificate has to contain; we build it to that spec.
Can you convert our paper forms and PDFs into ServiceM8 forms?
Yes, that is most of the job. You send the forms your team uses now, we rebuild them as ServiceM8 forms with the right question types and skip logic, and design the Word template so the output still looks like your paperwork.
Can a form be required before a job is completed?
Forms attach to jobs through badges, and each badge can be set as mandatory or optional. For a safety check or a certificate that has to exist on every job of a certain type, we set it up as mandatory on those jobs.
Should I buy from the Form Store or get a custom form built?
Check the Form Store first. If a ready-made form fits your questions and your layout, it is the cheaper answer and we will say so. A custom build is worth paying for when your trade, your certificate format or your branding does not match what is in the store.
